Drug Class: **NSAID** (Non-Steroidal Anti-Inflammatory Drug)

**Antiplatelet agent** (prevents platelet aggregation)

Acetyl stamp

Irreversible inhibition via **acetylation** of the COX enzyme

Indications

Traffic jams trapped by dirty, sticky delivery plates

Thrombosis, Myocardial Infarction (MI), and Stroke caused by platelet aggregation blocking blood vessels

Patrolling to stamp plates before jams start

**Prophylactic use** (primary and secondary prevention) for MI and Stroke

Mechanism of Action & Dosing

Cyclo-oxygenase 1 unicycle (1 wheel)

**COX-1 Inhibition**

Going slow / Low speed (\~100 mg/day)

**Low-dose Aspirin** (\~75–100 mg/day): Selectively targets COX-1 for antiplatelet effect

Permanently banned plates; replacement takes 7–10 days

Irreversible inhibition of platelets; life span of platelets is **7 to 10 days**

Cyclo-oxygenase 2 bicycle (2 wheels)

**COX-2 Inhibition**

Medium speed (200–2000 mg/day) for angry businessmen (red/fever, pain)

**Medium-dose Aspirin**: Analgesic (pain relief) and Antipyretic (fever reduction) effects

Maximum speed (2000–4000 mg/day) for city-wide inflammatory riots

**High-dose Aspirin**: Anti-inflammatory effect

Adverse Effects & Interactions

Special teeth on tyres scraping the protective mucosal lining

COX-1 inhibition reduces protective **gastric mucosal synthesis** (PGE2/PGI2), leading to **gastric ulcers**

Too many officers clearing plates making roads slippery

Concomitant use with other NSAIDs/anticoagulants increases **bleeding risk**

High-speed pedalling causing high-pitched ringing sound

**Tinnitus** (a classic sign of Aspirin toxicity / Salicylism)

Debris splashing onto water pipes, rusting and clogging them

**Tubulointerstitial nephritis** and decreased renal perfusion (kidney injury caused by chronic NSAID use)
