## Atropine: The Tale of A Trophy

Panel 1: The Enchanted Trophy Chalice & Chemical Structure

An ancient relic chalice named "A Trophy" is cursed to remain bone-dry, forged from 3 special metals that give it shine and allow it to penetrate deep into the soul's defenses.

**Drug & Structure:** "A Trophy" stands for **Atropine**. The dry chalice represents its core **anticholinergic** nature. Being forged from **3 Special metals (3S)** signifies it is a **tertiary amine**, allowing it to easily cross biological membranes and **penetrate the central nervous system (CNS)**.

Panel 2: Repelling the Musk Monster (Mechanism of Action)

A giant toxic mushroom monster tries to melt the Chalice with poisonous musk, but the Chalice projects a force field that repels it completely.

**Mechanism of Action:** The mushroom + musk monster represents **muscarinic agonists** (muscarine / muscarinic receptors). The Chalice's force field signifies that atropine acts as a competitive **antimuscarinic agent**, blocking acetylcholine at muscarinic receptors.

Panel 3: The Unaffected Nicotine Guards (Receptor Selectivity)

Muscular guards wearing nicotine patches pass right through the Chalice's force field completely unaffected.

**Receptor Selectivity:** The guards with nicotine patches represent **nicotinic acetylcholine receptors** (Nn and Nm). Atropine selectively blocks muscarinic receptors and **does NOT affect nicotinic receptors**.

Panel 4: Curses of Blindness & Heat (Mydriasis & Hyperthermia)

An adventurer enters and is instantly blinded by the Trophy's shine, while hot desert winds burn away his skin with intense heat.

**Toxicities (Blind & Hot):** "Blind as a bat" refers to **mydriasis** (pupil dilation via M3 blockage), **cycloplegia** (paralysis of accommodation), photophobia, and increased **glaucoma risk**. "Hot as a hare" signifies **hyperthermia** caused by the inhibition of sweat glands.

Panel 5: Curse of Extreme Dryness (Inhibition of Secretions)

Every drop of moisture is violently sucked out of the adventurer's body, leaving his tissues completely parched.

**Toxicities (Dry):** "Dry as a bone" represents marked **decreases in salivary, respiratory, and gastric secretions** (via M3 blockade), alongside side effects like severe dry mouth, **constipation**, and **urinary retention**.

Panel 6: Heart & Mind Alterations (Tachycardia & CNS Effects)

Blood surges to the adventurer's skin turning it crimson, his heart races wildly, and his mind is consumed by terrifying visions.

**Toxicities (Red & Mad):** "Red as a beet" represents **flushed skin** due to cutaneous vasodilation. Heart rate increases (**tachycardia**) via M2 blockage / decreased vagal tone. "Mad as a hatter" signifies CNS toxicity, causing **confusion, hallucinations, and seizures**.

Panel 7: Saving the City from Poison (Therapeutic Uses)

When green toxic smoke covers the city and citizens drown in their own fluids with failing hearts, the Chalice's power dries the poison and restarts their hearts.

**Clinical Indications:** Atropine is a primary antidote for **organophosphate poisoning** (pesticides/nerve agents) to dry lethal pulmonary secretions and treats severe **symptomatic bradycardia** by increasing heart rate.

Panel 8: Premedication & Specialized Uses

Alchemists invoke the Chalice before putting patients into deep sleep, while healers use tiny fragments to freeze eye muscles and calm overactive bladders.

**Other Medical Uses:** Used as **premedication before anesthesia** to prevent airway secretion buildup, as an **ophthalmic agent** for refraction exams/uveitis, and for treating **overactive bladder** / urological spasms.

Panel 9: Drug Interactions & The Furious Physician (Antidote)

Combining the Chalice with Trident-Crows or Ant-Hives drives elders mad, but a Furious Physician with steam pouring into his brain arrives to shatter the spell.

**Interactions & Antidote:** Anticholinergic burden increases when combined with **TCAs ("Trident-Crows")** or **Antihistamines ("Ant-Hives")**. Memory drugs (**Donepezil**) counteract its effects. Central atropine toxicity is reversed by **Physostigmine ("Phys-o-stigmine")**, a tertiary acetylcholinesterase inhibitor that **crosses the blood-brain barrier**.
