Panel 1: The Surfing Duo (Drug Identification & Class)

Two legendary friends known as the Glucose-Glazed Coats duo come to the beach for surfing.

This is a Glcocorticoid Steroid.

Panel 2: Calming the Ocean Floor (Mechanism of Action)

A big Tide pulled them down to the ocean floor, where they find angry, red fish rioting from chemical pollution. They need to think of something quickly.

Roiting, red organisms is the symbol for Inflammation.

Panel 3: Altering the DNA

Fortunately, one of them has the karyotype key to enter the fish's nucleus and alter DNA.

**Mechanism of Action:** Budesonide enters target cells and translocates into the nucleus to **alter DNA transcription and protein synthesis**, suppressing inflammatory cytokine production to halt **inflammation**.

Panel 4: The Reef Highway (Metabolism)

The grateful fish guide them through a liver-shaped reef highway garden that scrubs away dirt, dead cells, and bacteria.

**Metabolism:** Budesonide undergoes extensive first-pass **hepatic metabolism** primarily via the **CYP3A4 (CYP450) pathway**.

Panel 5: Deep Breaths and Chill Pills (Clinical Indications & Routes)

When the giant tide surges again, one buddy takes a deep breath in, while the other takes an oral chill-pill to stay calm.

**Clinical Indications:** Administered via **inhalation** for maintenance treatment of **asthma and COPD**, and as an **oral formulation** targeting localized gastrointestinal inflammation in conditions such as **Crohn's disease**.

Panel 6: The Veteran Surfer (Adverse Effects)

Back on shore, they meet a veteran surfer who spent too long in the tides: his face is visibly swollen from chronic sun exposure, and blowing sand has clouded his eyes.

**Adverse Effects (Chronic Use):** Long-term systemic glucocorticoid exposure can cause **cushingoid appearance (moon facies)** and ocular complications like **cataracts**.
