## 2nd Generation Cephalosporins: The Furious Younger Brother

Panel 1: The Furious Two-Spoon Brother (Drug Name & Class)

This is the story of the three-spoon viking's younger brother—a viking who wielded an axe with two chef's spoons at the end and was always angry and furious.

**Drug Identity:** Memory aid for **Cefuroxime** (furious + two spoons = **2nd generation cephalosporin**).

Panel 2: Striking the PBP Guards (Mechanism of Action)

He spots an unconscious person in the street and rushes over, knocking out the interfering PBP guards with a mighty swing of his two-spoon axe.

**Mechanism of Action:** Inhibits bacterial cell wall synthesis by **binding and blocking Penicillin-Binding Proteins (PBPs)**.

Panel 3: Fanning the Powdered Medicine (Respiratory Indications)

Using his two-spoon axe like a hand-fan, he blows medicinal powder directly into the patient's nose, pharynx, and lungs.

**Clinical Indications:** Commonly used for **upper and lower respiratory tract infections** (e.g., sinusitis, pharyngitis, bronchitis, community-acquired pneumonia).

Panel 4: Cracking Kidneys (Nephrotoxicity)

The powder enters the bloodstream and strains the kidneys so severely during filtration that they begin to crack under the pressure.

**Adverse Effects:** Potential for **nephrotoxicity** and acute kidney injury, particularly when cleared renally.

Panel 5: The Mud Puddle Fall (Gastrointestinal Side Effects)

Shocked by what is happening, the young viking loses his balance and lands flat in a sloppy, brown mud puddle behind him.

**Adverse Effects:** Frequently causes GI disturbance, notably **diarrhea and nausea**.

Panel 6: The Older Brother's Furious Smile (Drug Interaction)

His older brother watches in annoyance, flashing a stern, furious smile—though he relaxes once he sees the patient is saved.

**Drug Interaction:** Interaction with loop diuretics like **Furosemide** (furious-smile), which can amplify renal toxicity risk.
