Alteplase

Alteplase Visual Mnemonic Guide

Panel 1: Old-Te-Place & Fibrin Clots
This is the story of Old-te-Place Castle where thick fibers covered everything around the mansion.
Drug Class & Target: Alteplase (Old-te-Place) is a tissue plasminogen activator (tPA) classified as a fibrinolytic agent that targets fibrin clots (thick fibers) forming pathological thrombi.
Panel 2: The Inactive State (Plasminogen)
Everything is like this because there was a special plasma trapped inside a glowing Plasma-gem.
Zymogen Form: Represents circulating plasminogen (Plasma-gem), the inactive precursor enzyme trapped until activated by alteplase.
Panel 3: Mechanism of Action
One day, the plasma broke free from the plasma-gem, shooting powerful plasma rays that cut through and destroyed the surrounding fibers.
Mechanism of Action: Alteplase converts inactive plasminogen to active plasmin (plasma rays escaping), which directly breaks down and dissolves the fibrin mesh work of blood clots.
Panel 4: Clinical Indications
These powerful plasma rays flew across the realm to save and help people suffering from heart attacks, strokes, and blocked lungs.
Indications: Alteplase is indicated for the acute treatment and thrombolytic therapy of Myocardial Infarction (MI), acute Ischemic Stroke, and massive Pulmonary Embolism (PE).
Panel 5: Drug Interactions & Bleeding Risk
Problems and dangerous chaos arise if the War-Fairy or Clopping Clog join the fight, leading to severe bleeding.
Drug Interactions & Risk: Serious interactions occur when co-administered with anticoagulants like warfarin (War-Fairy) or antiplatelet agents like clopidogrel (Clopping Clog), significantly elevating the risk of life-threatening excess hemorrhage.
Panel 6: Contraindications & Reversal Therapy
It should be kept away from shady visitors showing surgical scars, as bleeding will begin. If hemorrhage occurs, the only solution is to pour fresh frozen plasma to stop it.
Contraindications & Reversal: Contraindicated in patients with recent surgery or trauma (surgical scars) due to risk of severe bleeding. In the event of severe, uncontrollable bleeding, emergency reversal includes administering Fresh Frozen Plasma (FFP) (or cryoprecipitate/antifibrinolytics) to restore clotting factors.
