Amiodarone
Amiodarone Visual Mnemonic Guide
Panel 1: Amy, The Daring One & The Thyroid Necklace
There was a young girl known as Amy: The Daring One. She had a special heart and a special necklace around her neck which gave her more superpowers, sitting right over her thyroid.
Drug Name & Structural Analog: Amiodarone (Amy, The Daring One) is a Class III antiarrhythmic agent that is structurally related to thyroxine (T4), wearing an iodine-rich "necklace" close to the thyroid gland.
Panel 2: Friends to All & Agitated Hearts
She was kind and friendly to everyone—even salt shakers, milk cartons, bananas, and beta-electron particles—and she loved to calm down agitated hearts.
Mechanism of Action: While primarily a Class III K+ channel blocker, Amiodarone has broad multichannel actions: blocking Sodium (Na+) channels (salt shaker), Calcium (Ca2+) channels (milk carton), Potassium (K+) channels (banana), and exerting non-competitive Beta-blocker activity (beta-electron particles) to act as an antiarrhythmic.
Panel 3: Poison Ivy into Medicine
She can turn Poison Ivy into a powerful medicine to cure agitated hearts and restore their rhythm.
Route & Indication: Intravenous (IV) Amiodarone (Poison Ivy) is indicated for the acute conversion and management of life-threatening cardiac arrhythmias, such as Ventricular Tachycardia (V. Tach) and Ventricular Fibrillation (V. Fib).
Panel 4: Unwanted Side Effects (Lungs & Thyroid)
Her powers came with a price: while calming hearts, she caused lungs to become fibrous and stiff, and her necklace altered the thyroid glands of those around her.
Adverse Effects: Major toxicities include Pulmonary Fibrosis (stiff, fibrous lungs) and thyroid dysfunction—causing either Hyperthyroidism or Hypothyroidism due to its high iodine content and structural similarity to thyroxine.
Panel 5: The CYP450 Highway Traffic Jam
The villagers asked her to go to the city to cure more people, but she unknowingly walked right down the middle of the CYP450 highway, causing a major traffic jam.
Metabolism & Enzyme Inhibition: Amiodarone is a potent CYP450 inhibitor (traffic jam on the CYP450 highway), slowing down the metabolism of many other co-administered medications and increasing their plasma concentrations.
Panel 6: The Quarrel & Bradycardia
She got into a fierce quarrel with the War-Fairy, and to cool off, Amy took a deep breath and slowed her heart rate down significantly.
Drug Interactions & Adverse Effect: Amiodarone strongly interacts with Warfarin (War-Fairy) by inhibiting its metabolism and dramatically increasing bleeding risks. Additionally, its suppressive effects on cardiac conduction can lead to severe Bradycardia (abnormally slow heart rate) and heart block.
